Service Agreement: Basic Plus Billing Package
1. Term and Fees The Client agrees to a minimum term of six (6) months. The monthly fee is $475.00), which provides ten (10) hours of service per month.
2. Scope of Work Allocated hours may be utilized for general insurance and billing tasks, including but not limited to:
Claims, adjustments, and reconciliation of remittance issues.
Patient invoicing, benefits verification, prior authorizations, and coverage analysis.
Portal registration and maintenance (Availity, NPPES, PECOS, CAQH, etc.).
3. Exclusions Credentialing and paneling services are strictly excluded from this package. These services require a separate consultation and fee schedule.
4. Overage and Rollover Policy
Expiration: Unused hours expire at the end of the month and do not roll over.
Additional Time: Hours in excess of the included 10 may be purchased at the Client's discounted package rate, subject to Provider availability.

This price is subject to a 6 month commitment. If you chose to stop the 6 month commitment please give 60 day notice and there will be a $250 early cancelation fee. If there are any late or returned payments then there will be a $50 additional charge added to your balance for that month.
