Service Agreement: Cohesive Billing Package
1. Term and Fees The Client agrees to a minimum term of six (6) months. The monthly fee is $1,250.00, which provides thirty (30) hours of service per month.
2. Scope of Work Allocated hours may be utilized for general insurance and billing tasks, including but not limited to:
Claims, adjustments, and reconciliation of remittance issues.
Patient invoicing, benefits verification, prior authorizations, and coverage analysis.
Portal registration and maintenance (Availity, NPPES, PECOS, CAQH, etc.).
3. Revenue Optimization The Provider will review the Client's use of CPT codes. If underutilization is identified, the Provider will advise on integrating additional applicable codes to potentially increase practice revenue. This training and consultation are included within the monthly hours.
4. Exclusions Credentialing and paneling services are not included in this package. These services require a separate consultation and fee schedule.
5. Overage and Rollover Policy
Expiration: Unused hours expire at the end of the month and do not roll over.
Additional Time: Hours in excess of the included 30 may be purchased at the Client's discounted package rate, subject to Provider availability.

This price is subject to a 6 month commitment. If you chose to stop the 12 month commitment please give 60 day notice and there will be a $750 early cancelation fee. If there are any late or returned payments then there will be a $50 additional charge added to your balance for that month.
