Service Agreement: In-Depth Billing Package
1. Term and Fees The Client agrees to a minimum term of six (6) months. The monthly fee is $2,000.00, which provides fifty (50) hours of service per month.
2. Scope of Work Allocated hours may be utilized for general insurance and billing tasks, including but not limited to:
Claims, adjustments, and reconciliation of remittance issues.
Patient invoicing, benefits verification, prior authorizations, and coverage analysis.
Portal setup (Availity, NPPES, PECOS, CAQH).
Code revision and revenue optimization training.
3. Credentialing and Paneling Election The Client may elect to utilize monthly package hours for credentialing services.
Resource Allocation Warning: The Client acknowledges that credentialing is time-intensive and may consume the majority of the monthly allotment (e.g., 40+ hours in a single month), significantly reducing availability for standard billing tasks.
Option: The Client may choose, upon consultation, to pay a separate fee for credentialing to preserve monthly hours for billing tasks.
4. Overage and Rollover Policy
Expiration: Unused hours expire at the end of the month and do not roll over.
Additional Time: Hours in excess of the included 50 may be purchased at the Client's discounted package rate, subject to Provider availability.

This price is subject to a 6 month commitment. If you chose to stop the 12 month commitment please give 60 day notice and there will be a $1000 early cancelation fee. If there are any late or returned payments then there will be a $50 additional charge added to your balance for that month.
