Service Agreement: Modern Billing Package
1. Term and Fees The Client agrees to a minimum term of six (6) months. The monthly fee is $900.00, which provides twenty (20) hours of service per month.
2. Scope of Work Allocated hours may be utilized for general insurance and billing-related tasks, including but not limited to:
Claims, adjustments, and reconciliation of remittance issues.
Patient statements, invoicing, and correspondence.
Benefits verification, prior authorizations, cost estimates, and coverage analysis.
Portal registration and maintenance (Availity, NPPES, PECOS, CAQH, etc.).
Staff training and billing code optimization.
3. Revenue Optimization The Provider will review the Client's use of CPT codes. If underutilization is identified, the Provider will advise on integrating additional applicable codes to potentially increase practice revenue.
4. Overage and Rollover Policy
Unused hours expire at the end of the month and do not roll over.
Additional hours may be requested at a discounted rate of $40.00/hour, subject to Provider availability.
5. Exclusions Credentialing and paneling services are not included in this package. These services require a separate consultation and fee schedule.

This price is subject to a 6 month commitment. If you chose to stop the 12 month commitment please give 60 day notice and there will be a $500 early cancelation fee. If there are any late or returned payments then there will be a $50 additional charge added to your balance for that month.
