Service Agreement: Off To A Great Start Package
1. Fees and Term The Client agrees to a one-time flat fee. The term of this agreement shall extend for one (1) month or until the utilization of eighty (80) service hours, whichever duration is longer.
2. Scope of Services The Provider shall perform the following duties:
Company re-credentialing and association with new Tax ID.
Submission of updated W9 forms and roster updates for in-network providers.
One (1) comprehensive coding and billing training session, including training materials.
3. Timeline Clarification The Provider acknowledges that third-party payer processing times vary. If contracting delays extend the project beyond one calendar month, the agreement remains in force until the full allotment of 80 service hours has been exhausted.
4. Large Group Surcharge The base fee covers companies with up to ten (10) employees. For companies exceeding ten employees, an additional fee of $100.00 will be charged for each additional provider requiring credentialing or paneling services. (processing fees are applied)

This price is subject to a 1 month or 80 hour commitment whichever is longer.. There is a one time payment of $3000.00. If there are any late or returned payments then there will be a $50 additional charge.
