Service Agreement: Individual Credentialing

1. Fees and Promotional Pricing The Client agrees to a base fee of $295.00 per insurance panel (e.g., Medicare, Medicaid, Aetna, Cigna, etc.).

  • Volume Incentive: For every two (2) panels purchased at the full rate, the Client is entitled to one (1) additional paneling service at no charge (Buy 2, Get 1 Free).

  • Condition: To qualify for this incentive, you must be an individual provider contracting as an individual not as a group, all panels (both paid and promotional) must be selected and contracted simultaneously upfront. Panels added at a later date will be charged at the standard rate.

2. Client Obligations Intake Session: The Client agrees to schedule and attend an initial 30-minute consultation to verify that all necessary documentation is on hand prior to the commencement of the credentialing process. (processing fees are applied)

Contracting & credentialing is the process of submitting the application accurately to the insurance of your choosing and obtain a tracking number or proof of submission. If a credentialing or contracting process is not signed in time, failed due to lack of response from provider and the process must start again then the fee will be assessed again.
