Individual Credentialing Services
Individual credentialing is required for providers who are newly licensed, joining a practice, being added to an existing group roster, or seeking participation with insurance networks for the first time. Most insurance carriers require providers to complete the credentialing process before they may bill as an in-network provider.
The Insurance Magician, LLC will assist with the preparation, submission, and monitoring of credentialing applications with participating insurance carriers.
Client Obligations
Initial Consultation
The Client agrees to schedule and attend a 30-minute consultation to determine enrollment goals, identify target insurance carriers, and review all required documentation.
Provider Documentation
The Client shall provide the following:
A current and freshly attested CAQH ProView profile.
A copy of the provider's current state license.
Proof of current professional liability/malpractice insurance coverage.
A completed and up-to-date Provider Data Summary in PDF format, including all current and previous employment information, practice locations, education, training, and professional history.
Any additional documentation requested by the insurance carrier during the credentialing process.
Service Fee
Individual Credentialing Fee: $295.00
Processing fees may apply.
Important Notice
The Insurance Magician, LLC will use commercially reasonable efforts to prepare, submit, and follow up on all credentialing applications. However, approval, participation status, effective dates, reimbursement rates, and processing timelines are determined solely by the insurance carrier.
The Insurance Magician, LLC cannot guarantee acceptance into any insurance network, approval of any credentialing application, specific reimbursement rates, or payer processing timelines. All fees paid are for services rendered and are non-refundable once work has commenced.
